Walkthrough

Enter Havenreach Town Hall (door at (41, 14) in Havenreach), talk to Nessa Vale, choose The First Ledger and Accept quest.
Search all three spots in the Town Hall, in any order: the shipping ledgers at (1, 1), the council records at (13, 1) and the old document crate at (14, 6). Read each to the end.
Talk to Bard Liora around (51, 36) in Havenreach, choose The First Ledger and then Continue.
Talk to Sister Elyra around (31, 11), choose The First Ledger and then Continue.
Talk to Nessa. Place the fragments in this order: MEMORY, then NAME, then OATH. A wrong choice starts the arrangement over. When it is complete you receive the Fragment of the First Ledger.
Talk to Nessa again and choose Complete quest.
Notes
- Keep the Fragment of the First Ledger: the next three chapters need it. If you lose it, Nessa offers Recover missing story items.
